Cancellation Policy

ERx Vitality ™️ reserves the right to hold a No Refund and No Return policy on all purchases made.


Cancellation request: Patients who wish to cancel their treatment plan for any reason must provide written request within 30 days of payment processing at: Cancellation Request. Patients will be charged for the following month is cancelled outside of the 30 day window. 


Prescription purchase: Patients expressly agree and acknowledge that ERx Vitality (or a 3rd party payment processor) is authorized to charge the account the price of the chosen plan aforementioned during the comprehensive assessment in addition to any associated  charges that were decided on during the intake process.  


Prescription refills:  Patients are required to follow the link on the website listed:  Medication Refill for any and all prescription refill requests. Medications will not be automatically ordered or distributed by ERx Vitality without first contact from patients as above. Any delay in treatments is  placed on the responsibility of the patient and no refunds or discounts will be made by ERx Vitality. 


Termination with non-payment: Patients agree that any treatment plan purchased will be charged for the given time frame as agreed upon during the intake process. ERx Vitality reserves the right to terminate treatment plans at any time due to acount deliquency. ERx Vitality will not be held liable for any damages caused by such terminations. 


Collection costs:  Patients agree to pay all fees associated with collection from deliquent accounts.
